The Georgia Office of EMS and Trauma uses an online platform to process all initial and renewed physician and instructor license applications. Paper applications are NOT accepted. EMS agencies will use the system to add staff to their roster, add / change authorized agents or medical directors, submit renewal requests, and much more. Hospitals will use the system to apply for designation as a trauma, stroke or cardiology center.

To ensure you receive emails from the new license management system, be sure to add “[email protected]“to your list of safe senders and recipients in your email client.

  • How to get your account information for current and previous licensees (doctors / instructors)

    If you are a doctor with an existing license, you already have an account in the system, so please follow these steps to get your account information:

    1. Go to
    2. Scroll down to the bottom and click Forgot username (do NOT create a new account!)
    3. You will then enter your date of birth, SSN, and last name, and click Search
    4. If your DOB / SSN / Last Name matches our license records, you will see that an account was found, but it does not have an email. Please enter your email address. We recommend that you use your personal email address either,, and, as other domains like icloud and business email systems end up blocking emails from LMS.
      • NOTE: If you enter your email address incorrectly, you will need to contact the Office of Trauma and Emergency Medical Services at 770-996-3133 to correct your email address. You will need to provide your name, social security number, date of birth, and address in order for staff to correct your email address.
    5. You will then receive an email from LMS with your username and a link to reset your password, which must be set immediately.


  • How to create an account if you are NOT a current license holder

    For new staff (who have never been laid off), follow these steps to create an account:

    1. Go to
    2. Click the Create Account button and follow the steps to create an account.
      • Please pay particular attention to the minimum set of information you must provide if you intend to apply for a license as a doctor or instructor in the future, including first name, last name, date of birth , SSN, email, address, etc.
      • Users must use a personal email address when creating an account. The best domains capable of receiving e-mail from the system are,, and Other domains like iCloud and others seem to have problems allowing system emails. System emails originate from [email protected].


  • Generation of new license cards for existing doctors / instructors

    Currently licensed physicians and instructors who wish to generate a new license card can generate them at any time on LMS by doing the following:

    1. Go to and login (see instructions above for how to access the system)
    2. Click on Applications, then on “Generate Medic License Card & Email” or “Generate Instructor License Card & Email”
    3. The user will receive an email with the card and the document will be available on the LMS under “My account”, then “Documents”
